2/29/2024 EnchantaVaughn Approved 1.Behavior: Ms. Bates reported that Jeremiah go into a verbal confrontation with several family members last night over a disagreement with a family member. Ms. Bates reported that Jeremiah continues to exhibit at-risk behaviors within the home and community. Ms. Bates reported that he was being disrespectful and loud after given instructions to stop turning the television .Jeremiah exhibited these behaviors for up to one hours inconsistently and has had difficulty focusing at least 3-5xs weekly for the past 30 days. 2.Behavior: Ms. Bates reported that Jeremiah became angry and upset with his prior to QMHP-C arrival for todays. Ms. Bates reported that Jeremiah stated “I it in this house”. Ms. Bates went on to report that Jeremiah has serious issues dealing his disrespectful behaviors, Jeremiah exhibited these behaviors for up to two hours inconsistently and has had difficulty focusing at least weekly for the past 30 days. 1.Intervention: QMHP-C assessed and monitored Jeremiah’s mood and behavior. QMHP-C probed in order to process and explore Jeremiah’s poor communication skills used with his parents and others. QMHP-C reviewed and discussed effective ways to communicate with others when provoked or angry within the home and community. QMHP-C provided Jeremiah with note card to assist his with brainstorming techniques to assist his with decreasing his behaviors within the neighborhood. 2.Intervention: QMHP-C reviewed the importance of trying to meet goals and objectives with Jeremiah. QMHP-C asked questions about reported behaviors within the home. QMHP-C allow Jeremiah to ask questions along with expressing his feelings and concerns. QMHP-C and Jeremiah reviewed and discussed anger management techniques. QMHP-C provided Jeremiah with feedback along with demonstrating model techniques to utilize when provoked or felling angry or depressed. 1.Response: Jeremiah stated that he was upset and didn’t feel like hearing or seeing his family members who live with her. Jeremiah was observed being disrespectful to his parents during todays session. Jeremiah continue to talk back towards Ms. Bates while talking to QMHP-C. Jeremiah went on to report that he doesn’t like everyone teaming up on his when it comes to his family members. 2.Response: Jeremiah admitted that he was a little disrespectful and stated he didn’t care. Jeremiah stated that he will talk to QMHP-C later about some issues dealing his relationship with his dad. Jeremiah stated that he will just stay to herself and call his friends later on discuss homework assignments. Jeremiah reported that he will look over notes after today’s session.
